Diet and Exercise Not Working? When to Consider Medical Support

Weight & Metabolic Health

Written by the Veritas Pharmacy clinical team. Address: 113 Chester Road, Sunderland. Registered with the General Pharmaceutical Council, registration number 9012617.

Most people who struggle to lose weight have already tried diet and exercise, often more than once, often with real effort behind it. When it still does not shift, the usual response is to blame willpower. Often the real explanation is metabolic, hormonal, or behavioural, and no amount of discipline fixes those on its own.

Why diet and exercise alone sometimes is not enough

Sustainable weight management depends on identifying what is actually driving weight gain, not just restricting calories. Thyroid function, insulin resistance, medication side effects, sleep quality, and stress hormones can all work against consistent effort, quietly limiting results no matter how disciplined the approach. This is exactly why a clinical assessment, rather than another diet plan, is often the missing step.

Who should consider a medically supervised approach

A structured, clinically supervised programme is generally appropriate for adults with a BMI of 30 or above, or a BMI of 27 or above alongside a weight-related health condition such as type 2 diabetes, hypertension, or sleep apnoea. It is also worth considering if you have made genuine, repeated efforts through diet and exercise alone without sustained results.

Medication is one option, not the default. Many patients see significant results from structured behavioural support and nutritional guidance alone. Where clinically appropriate, medication options including GLP-1 receptor agonists can be discussed, but only after a full clinical assessment, and only as one part of a wider plan.

What a clinical assessment actually involves

A proper assessment goes well beyond stepping on some scales. The initial consultation includes BMI, blood pressure, and blood glucose measurement, and a full review of your health history — this consultation itself is free of charge. Where baseline or ongoing blood testing is recommended to check metabolic markers, thyroid function, and liver function, this is a separately charged service, priced at £99 plus a £35 phlebotomy fee for the dedicated Weight Loss Management Profile, which covers 20 markers including liver and kidney function, cholesterol, blood sugar control (HbA1c), and pancreatic enzymes. This identifies medical factors that a generic diet plan would never catch, and gives a genuine starting point rather than a guess.

From there, a personalised plan is built around your circumstances, with regular follow-up appointments to review progress, adjust the approach, and monitor safely if medication forms part of the plan. Clinically meaningful weight loss, generally 5 to 10 percent of body weight, is typically achieved over 3 to 6 months with consistent engagement.

Free consultation, charged testing. The initial clinical assessment consultation is free. Blood testing — whether as a baseline before starting, or ongoing monitoring while on GLP-1 medication — is priced separately. See the Weight Loss Management Profile for full pricing and what’s included.

Frequently asked questions about medical weight management

Will I be put on medication straight away?
No. Medication is only prescribed where clinically appropriate after a full assessment. Many patients benefit significantly from structured behavioural support and nutritional guidance alone.
Do I need blood tests before starting, and are they included?
Baseline blood tests are recommended to check metabolic markers, blood glucose, thyroid function, and liver function before starting. The initial consultation itself is free, but blood testing is a separately charged service — the dedicated Weight Loss Management Profile is £99 plus a £35 phlebotomy fee.
Is the initial consultation free?
Yes, the initial clinical assessment consultation is free of charge. Where blood testing is recommended as part of your assessment or ongoing monitoring, this is a separately charged service.
Are GLP-1 medications available?
The prescribing pharmacist can assess eligibility for GLP-1 receptor agonists as part of a medically supervised programme. Eligibility depends on BMI, health history, and other clinical criteria.
How long does the programme take?
Clinically meaningful weight loss, generally 5 to 10 percent of body weight, is typically achieved over 3 to 6 months with consistent engagement. Total duration depends on individual goals and response.
