The BCG vaccine protects against tuberculosis and is offered to people with an increased risk of exposure, including some university students, healthcare and laboratory workers, and those with family or travel connections to higher-incidence countries. Why a TB test usually comes first.
BCG is a live vaccine that helps protect against tuberculosis (TB), particularly its most severe forms. It was once given routinely to secondary school children in the UK, but this stopped in 2005 as general population TB rates fell. Today, it’s offered selectively to people at higher risk of exposure, based on factors such as occupation, family background, or where they’ll be living or studying.
Because BCG contains a live, weakened form of a TB-related bacterium, giving it to someone who already has a TB infection can cause a significant local reaction. For this reason, anyone without a clear BCG scar or reliable vaccination history is usually assessed with the Mantoux skin test first, as this is the test recommended in UK guidance ahead of BCG. A TB blood test (IGRA) may be used in some circumstances, but a negative result on its own is not automatically treated as clearance to proceed. Your pharmacist will confirm which test is right for you and review your result before your BCG appointment is booked.
Is the BCG vaccine right for you?
Some universities, particularly for medical, nursing and other clinical courses, ask for evidence of BCG vaccination or immunity.
Those under 35 with a recognised occupational TB exposure risk, following an appropriate risk assessment.
Particularly for extended stays, work, or study in countries with a TB incidence of 40 or more cases per 100,000 people.
Including close contact with someone with active TB, where advised following assessment.
BCG is most often offered to those under 35, but it can still be considered for adults over 35 in roles with a higher risk of TB exposure, following an individual risk assessment. Your pharmacist will discuss whether it’s appropriate for your situation.

How the BCG pathway works.
Check for a BCG scar or documented history
If you have clear evidence of a previous BCG vaccination, you may not need a test or repeat dose.
TB test first, if needed
Without a clear scar or history, you’ll usually need a Mantoux test first, as this is the test recommended before BCG. Your pharmacist reviews the result and confirms you’re suitable before your vaccination is booked.
Single injection
A single dose, given as a small injection just under the skin, usually in the upper arm. It’s given using a specific technique, into the skin rather than the muscle.
A local reaction is expected
A small blister at the injection site is normal, and may become a small sore over several weeks before healing, leaving a small scar. Your pharmacist will explain what to expect and when to seek advice.
