TB screening paperwork rarely explains itself. A new job, a nursing course, or a visa application might simply say “TB test required,” leaving the actual choice — and sometimes it is a choice — unclear. Here is how the two main tests differ, and how to work out which one your paperwork actually needs.

Why TB screening is required so often

Tuberculosis bacteria can lie inactive in the body for years without causing any symptoms, a state called latent TB. Screening checks for this inactive infection long before it could ever cause illness, which is why it is commonly required for starting a new job in healthcare or social care, enrolling on a university or nursing course, applying for a UK visa, or preparing to travel or work somewhere TB is more common.

TB IGRA blood test vs the Mantoux skin test

Both tests check for latent TB, but they work differently. The **TB IGRA blood test** needs a single blood sample, with results usually back within a few days — often the fastest route for work, study, or visa requirements, and it needs only one visit. The **Mantoux skin test** is a small injection under the skin of the forearm, checked in person two to three days later, so it needs two visits a few days apart. Some employers or visa categories name one test specifically by name, so it is always worth checking your paperwork before booking.

Not for active symptoms. A persistent cough lasting three weeks or more, coughing up blood, unexplained weight loss, night sweats, or ongoing fever are not screened privately. Contact your GP or NHS 111 promptly, as active TB needs urgent NHS assessment and treatment.

If you need the BCG vaccine too

The BCG offers added protection against severe TB for those at higher risk. Because BCG is a live vaccine, the Mantoux skin test is usually needed first, as this is the test recommended in UK guidance ahead of BCG — unless you already have a clear BCG scar or documented vaccination history. A clear screening result does not automatically mean the vaccine goes ahead; the pharmacist reviews the result and confirms suitability before BCG is booked as a separate appointment.
